Title: Day Break CNA
Location: 241 Smith Street, Sheridan, WY 82801
Reports to: Day Break Supervisor

ESSENTIAL JOB FUNCTIONS
Participant Care & Support:

  • Assist with activities of daily living (ADLs), including mobility, toileting, grooming, and personal hygiene, to support participants in maintaining dignity, comfort, and personal well-being.
  • Offer attentive supervision and support to ensure participant safety while promoting independence and confidence in daily routines.
  • Assist participants during meal service and provide hydration reminders to help maintain proper nutrition and overall health.
Engagement & Social Well-Being:
  • Encourage and assist with group and individual activities that foster social interaction, meaningful engagement, and a positive program experience for participants.
  • Build respectful and supportive relationships with participants to promote inclusion, participation, and emotional well-being.
Health Monitoring & Communication:
  • Observe participants for changes in physical condition, behavior, or emotional well-being and promptly communicate relevant information to Day Break Supervisor to ensure timely support and intervention.
  • Maintain clear and professional communication with team members, participants, and families to support coordinated and participant-centered care.
Training and Compliance:
  • Completes comprehensive orientation and ongoing training as directed by the organization.
  • Proficient and willing to perform CPR and Basic First Aid.
  • Annual Tuberculosis (TB) assessment is mandatory.
  • Maintains strict confidentiality regarding sensitive information.
Environment & Program Operations:
  • Maintain a clean, organized, and safe environment that supports participant comfort and reduces potential safety risks.
  • Accurately document care provided and observations in accordance with program policies to ensure compliance, continuity of care, and effective team communication.
  • Additional tasks and responsibilities as needed to support the organization's goals and operations.
Knowledge, Skills, and Abilities:
  • Coachable with a willingness to learn and adapt to a changing workflow.
  • Basic nursing principles, personal care, and infection control as a Certified Nursing Assistant (CNA).
  • Understanding of adult development, aging, and the needs of dependent adults.
  • Awareness of techniques to engage participants in social, cognitive, and recreational activities.
  • Effective communication and people skills to engage participants and work collaboratively with staff.
  • Ability to provide safe assistance with personal care, grooming, mobility, and daily activities.
  • Accurate documentation and timely reporting of health, safety, and behavioral concerns.
  • Observation and assessment skills to identify changes in participant health or behavior.
  • Effective time management to support multiple participants and daily program routines.
  • Work effectively as part of a team to deliver high-quality care.
  • Adapt to the diverse needs of participants and changing daily routines.
Physical Requirements:
  • Demonstrates a full range of physical motion as required by the tasks.
  • Regularly lifts, transfers, and positions objects weighing up to 50 pounds without assistance.
  • Utilizes mechanical lifts to lift objects weighing more than 100 pounds.
Minimum Qualifications:
  • 21 years of age.
  • Current State of Wyoming Certified Nursing Assistant (CNA) license.
  • CPR certification.
  • Experience working with older adults or people with disabilities preferred.
  • Have a valid driver’s license and a safe driving record.
  • Employment is dependent on ability to pass a Central Registry and Division of Criminal Investigations background check.
