JOB TYPE: 40 hours/week (Full Time Position)
POSITION SUMMARY:
This role is responsible for providing medically prescribed, goal-oriented physical therapy in a patient’s home or select location based on treatment/patient needs. The role includes providing therapeutic treatments and participating in the development of patient physical therapy plans of care.
PRINCIPAL ACCOUNTABILITIES:
Perform initial and ongoing physical therapy evaluations and re-evaluations according to APTA standards of practice as well as state standards.
Develop plan of care/treatment plan for each patient based on the initial evaluation and objective findings including short and long-term goals, keeping the patient and caregiver as the focus of care.
Implement treatment plan on an established schedule and adjust this schedule and plan according to patient progress.
Document patient progress according to approved medical documentation and facility documentation procedures.
Evaluate mobility, strength and current level of patient function, while setting goals to improve in these areas that are scaled to patient abilities and care realistically attainable.
Report patient progress to patient, patient caregivers and healthcare professionals involved in ongoing treatment/care of patients.
Develop a plan for patients following completion of their Plan of Treatment with appropriate support and follow-up services if needed.
Maintain professional and technical knowledge through attending educational workshops, reviewing professional publications, etc.
QUALIFICATIONS:
Compassionate and able to deliver quality patient-centered care
Excellent problem solving, interpersonal and communication skills
Motivated and able be excel in a self-managed environment
Desire to work a flexible schedule based on patient needs
Ability to travel to patient homes or desired location for treatment
EDUCATION:
Bachelor’s Degree in Physical Therapy from an accredited physical therapy program.
CERTIFICATIONS, LICENSES AND REGISTRATIONS:
Currently-licensed Physical Therapist in the state of Wyoming
Possesses and maintains a current CPR certification
Valid driver’s license and auto liability insurance
