JOB DESCRIPTION
Title: Day Break Manager
Location: 211 Smith Street, Sheridan, WY 82801
Reports to: Director of Operations
ESSENTIAL JOB FUNCTIONS
Program Oversight & Compliance
- Oversee Day Break in accordance with State of Wyoming Standards for Adult Day Care.
- Develop and implement programs to meet client needs.
- Participate in program development.
- Create work plans and evaluate program outcomes.
- Assure that services are provided with safety, quality, and efficiency as the primary goals.
Participant Care & Support
- Responsible for the development and implementation of a Plan of Care for clients.
- Assess progress and functioning of clients.
- Orient new clients to the program and provide personal care services to clients.
- Oversee discharge planning.
- Address client concerns with other department heads.
- Reports health and safety concerns immediately.
Staff Supervision & Team Development
- Provide human resource management and supervision to Day Break staff and volunteers.
- Organize team development and training for staff and family members.
- Conduct annual evaluations and sets team goals.
- Communicates respectfully and builds rapport with clients and team.
Administrative & Operational Tasks
- Schedule Day Break Activities.
- Arrange meetings as necessary for the effective delivery of services.
- Complete required documentation daily in line with policies.
- Maintain confidentiality and participate in required training.
- Report monthly service units, prepare billing, and state report documents before submission.
Financial Management
- Monitor program budget to ensure ongoing financial stability.
- Participate in the annual planning and budget development process.
- Participate in fundraising and development activities to support program sustainability and community outreach.
Internal & External Duties
- Function as an ambassador for The Hub on Smith and the Day Break both inside and outside of the organization.
- Work directly with family members on an ongoing basis to assure effective communication.
- Update staff and public on community resources and services.
- Conduct community meetings and fosters partnerships.
- Collaborate with staff (coordinators, directors, managers, and the like) daily to ensure the goals and objectives of the organization are met.
- Attend meetings and collaborates with internal departments to support and achieve organizational goals.
- Periodically required to travel utilizing personal vehicle.
- Additional tasks and responsibilities as needed to support the organization's goals and operations.

Physical Demands
- Must be able to lift 50 pounds occasionally to assist participants or move equipment and supplies.
- Requires frequent standing, walking, bending, and reaching throughout the day.
- Ability to assist participants with mobility, including transferring in and out of chairs or wheelchairs.
- Must be able to respond quickly in emergency situations, including assisting with evacuations.
- Prolonged periods of sitting and computer use required for documentation and administrative tasks.
- Must be able to drive and operate a personal vehicle for work-related travel.
- Ability to speak clearly and listen actively in both one-on-one and group settings.
- May be exposed to bodily fluids, cleaning agents, and unpredictable behaviors associated with client’s cognitive decline or physical impairment.
Minimum Qualifications
- Bachelor’s degree in human services, Social Work, Nursing, Gerontology, or a related field; equivalent experience may be considered.
- Minimum of 5 years of experience working with older adults or individuals with disabilities, preferably in a day program, healthcare, or community services setting.
- At least 2 years of supervisory or leadership experience, including staff management and program oversight.
- Valid driver’s license and access to a personal vehicle for work-related travel.
- Current CPR and First Aid certification or ability to obtain upon hire.
- Strong written and verbal communication skills.
- Proficiency in Microsoft Office and ability to learn program-specific software with ease.
- Ability to pass a background check and meet all regulatory requirements for working in an adult day care setting.
